Obstetrics is the field of study concentrated on pregnancy, childbirth and the postpartum period. As a medical specialty, obstetrics is combined with gynecology under the discipline known as obstetrics and gynecology (OB/GYN), which is a surgical field.



My experience over Obstetrics "Pelvic pain/abnormal ultrasound results—specialist not concerned"

Would love some feedback on a specialist’s view of my pelvic ultrasound… In December I stopped taking oral contraceptives. Since then I’ve been experiencing painful periods, but they are regular (every 25-26 days, last for 3-4 days, and no excessive bleeding). I’ve also noticed pretty chronic pelvic pain that is worse when I’m lying down, especially when I’m on my side—It feels like the cramping I experience during my period. Because of this I saw my gynecologist last month who ordered a pelvic ultrasound that came back with these results:

PELVIC ULTRASOUND (WITH TRANSVAGINAL EXAM): INDICATION: 35 year old female. N9410: Unspecified dyspareunia LMP: 07/02/2022. COMPARISON: No previous films are available for comparison. TECHNIQUE: Sonographic evaluation of the pelvis was performed with transabdominal and transvaginal technique. The transvaginal examination was performed to better evaluate the endometrium, myometrium and ovaries. FINDINGS: * UTERUS: 5.7 x 3.0 x 4.6 cm in size. * MYOMETRIUM: Heterogeneous echogenicity. Small cystic structure in the fundal portion of uterus measuring .6 x .4 x .5 cm. Cervix was heterogenous with complexities as well as calcifications and Nabothian cyst. * ENDOMETRIUM: 7 mm. Normal echogenicity. Small amount of fluid within endometrium. * RIGHT OVARY: 3.9 x 1.4 x 1.6 cm in size. Normal echogenicity. * LEFT OVARY: 1.9 x 1.5 x 1.8 cm in size. Normal echogenicity. * OTHER FINDINGS: None. IMPRESSION: 1. Heterogeneous echogenicity of the uterus noted. 2. Small cystic structure in the fundal portion of uterus measuring .6 x .4 x .5 cm. 3. Cervix was heterogenous with complexities as well as calcifications and Nabothian cyst. 4. Endometrium measures 7 mm in thickness. Small amount of fluid within the endometrial cavity noted. 5. Ovaries are normal.

My gynecologist suggested endometriosis or adenomyosis and referred me to a specialist. I finally had an appointment with the specialist today who said he thought the cystic structure was “incidental” and said he does not believe it’s adenomyosis because there would be more of these structures present if so. He said he was not sure what to make of the cervix results which didn’t reassure me at all. He said my pain could be caused by endometriosis but can’t give a for certain answer unless I have surgery which he doesn’t want to suggest at this point. He suggested trying the pill again to see if it helps with the pelvic pain. The appointment ended with him saying he didn’t see anything too concerning on the ultrasound.

Any ultrasound techs or gynecologists out there have an opinion on the ultrasound itself or the specialist’s thoughts? I don’t know if it’s just my anxious brain getting to me, but the ultrasound results seem so much more abnormal/concerning then he made them out to be. I also have a grandmother who had uterine cancer so that cystic structure in my uterus is particularly worrying me.
